It has to do with changes in temperature & water erosion. All of teh substances they use expand & contract with the temperature. Then water, (& ice , snow in some areas) gets in & the traffic contributes. Probably someone can get more technical, that is basically it..
Usually it’s the freeze and thaw cycle constantly widening tiny cracks in the road surface until it breaks up. Can also be caused by heavy trucks dropping down a fraction of an inch after hitting a slight bump in the surface.
Years ago a huge pot-hole in Philly caused a bus to break an axle. The headline on news radio said the bus had fallen into a pot-hole. The newscasters then started riffing on how the bus was running a regular schedule from one end of the pot-hole to the other.

In Florida we have a limestone base and the underground water eats away the base and now we have a sinkhole.
