That actually happened at the church I attended many years ago. A member was in the attic and stepped off the rafters. And his foot went through the ceiling. Fortunately he wasn’t injured, only the ceiling.
The big lecture theaters in the university where I worked has plaster ceilings with narrow catwalks accessing storage rooms above. You didn’t want to step off the catwalks — it’s 40 feet to the floor!
