Thepatient says, “What’s that for?” The doc answers, “My advice.” The patient hands him $2 and says, “Take this, that’s my advice.” By the way the doctor’s name was an inside joke. Smith and Dale were both Jewish and Kronkheit is a Yiddish term meaning sickness.
