My first eye test in Missouri the lady insisted I take it with my glasses off:

ME: I don’t understand you.
HER: (Slower, finally) You see a 3D picture of a bridge. Along the bridge there are white rectangular signs, in each sign is a black diamond, and in each diamond is a white circle on the top, bottom, left or right.
Now, tell me where the dot is in the first sign.
ME: Where’s the bridge?
HER: You can put your glasses back on.
My uncorrected vision is 20/400. Fortunately, corrects to 20/25.

Plus you get something to dull the pain. Next time you have to go to renew your license, try taking some vicodin or codeine first. Not sure you will pass the test, but at least it will be relatively pain free.
