Things this bad have happened. The Genesis spacecraft had an accelerometer installed backwards. The Mars Climate Orbiter burned up in the atmosphere of Mars due to a failed Imperial/metric units conversion. The Soviet probe Venera 14 was supposed to measure the compressibility of the soil of Venus, but a lens cap happened to fall in just the spot where its arm was going to take the measurement.

This happened to one of the Ranger lunar probes, Ranger 6, I believe. Ranger 7 was the first one to successfully complete its mission.
