While responsibility does eventually, as Truman said, stop at the top, the real problem is a system developed long ago, that allows those in “middle management”(mostly political appointees initially) to CYA, and manipulate their performance appraisals. Yes, some things, like phony wars, ARE directly the responsibility of the top, but when it comes to the “day to day problems”, a much more complex critter is at large. Yes, it needs to be trapped and removed, but if a single entity at the top can stop it all, why does disease still exist despite an all-seeing, all knowing, “God”???
