See Wally in Dilbert as a benign example, or the "some bookkeeper arrested for diverting money to feed his or her habit news that we see several times a year. Add the “attractiveness” factor for folks who have a strong urge to prey on the particular organization’s clients (or workers) and the problem just gets worse… see also the Catholic Church’s ongoing pedophile epidemic.
The answer seems to be a combination of ethics (which can be taught, at least to most children), a good whistle-blower system, and surprise inspections. All of which depend on ethical, careful and hard working people at every level. Which ain’t inexpensive. Probably, the only way to ensure ethics at the top is to pay only an ethically appropriate salary, thus eliminating folks whose motives aren’t “pure”.
Or, of course, you could just stick to small organizations. Which have many of the same problems and the issue of doing local-only funding.
