Most of McCoy’s smaller medical instruments were salt and pepper shakers purchased for that episode. They wanted something futuristic looking, but the plot hinged on people knowing the vampire craved salt. Once they got the shakers, they looked too unusual and the director was afraid no one would realize what they were. So, they used a plain old salt shaker and the other things were repurposed as medical instruments.Once read that in a book about Trek.
