I had a similar situation about a year ago: While visiting a family member in the hospital (and relying on a cane due to a recent injury), I managed to step with my cane tip going right through the gap between regular floor and elevator floor! I barely caught myself on the wall & kept from wiping out! :-P

Based on reality – the vast majority of accidents involving elevators happen when going thru the doors.
