Joe, Comics characters can go two ways, they can grow up and get old or stay forever young and ideally “cute”.
It’s a problem, which would you prefer? And please allow for a few slip ups now and then.
Doug being adopted seems an awfully complex plot devise for such a simple and straightforward strip. It is much more likely that the creator of the comic did a “kid trick” to make Doug the age he is… and will be for the duration of the strip.
I also think that if Doug had been adopted, it would say so somewhere.

it’s really cute to see Sophie on Doug’s lap with Doug’s hand on her head. love the relationships between boy and his dog (just like Red and Rover!)
