    I doubt that anybody at the Post (or many anywhere in NYC, for that matter) knows that Missouri exists, much less Branson.

    That ‘butterfly form’ may be the only part of the ACA that actually does anything to limit health care costs.

    There are two ways of allocating (i.e., rationing) economic goods, like health care.
    One is by prices, either discovered through the action of free markets (very seldom seen nowadays) or set by government through some political process.
    The other is by direct allocation, what people usually have in mind when they talk about rationing. That can be either by a rule based system or some kind of panel of ‘experts’. Even a rule based system usually has some kind of appeals process where the ultimate decision is made by a group of individuals, and in any case the rules are made by some group. To my mind, using ‘death panel’ for the people in an ACA-type system who decide that A will get an MRI while B has to wait, and B dies of what he would have been tested for if allocation of MRI time were price-based, isn’t too much a stretch.
