I think Jade referred to the language in the policy that says that the US may use nuclear weapons first if attacked by a country that does not abide by the NPT such as North Korea or Iran.
Technically, mind you, it remains to be proven that Iran does not follow the NPT - there has not been any presented proof they are trying to make nuclear weapons.
