I have, on rare occasions, refused a bottle of wine. Not because it was a bad wine, but because it wasn’t to my taste. But in those cases, it was usually the waiter who had recommended a certain wine and gave me a bit to taste.
The “taste” is only to confirm the temperature. You do not get to open a whole bottle and say “that’s not what I meant” without paying for the entire bottle. However, I once returned a bottle of wine because it was no longer wine but was some type of cleaning fluid. The sommelier agreed and replaced it.
Heh, when in France in the early Sixties, my old man gave a thumbs down just to see what would happen. Waiter walked to the curb and poured the whole bottle into the gutter! My dad was SO embarrassed!

The real trick is to roll the cork between thumb and forefinger — mealy/crumbly cork = bad bottle/wine. The server should present the cork for approval (if he/she knows their stuff..

Wine can go bad in the bottle. It’s not actually the wine going off — sometimes the cork goes bad.
