Yes, it seems I fill out the form then in the examination room I get asked the same questions. All that stuff I assume would already be in my records. Granted sometimes there is something new, but not very often.
It’s ridiculous, and a waste of time to keep repeating the same…when they’re looking at the computer with all the info on it. Last time I had an appt, I was handed a clipboard with 5 sheets to answer. I was, to say the least very annoyed! I asked the doc why this long list? He said it’s not from my office but govt survey….and the last question I hit the roof and hollered at my doc this is ridiculous….IF I am ever handed this sort of survey again, I am refusing it! They can take it and shove it. Talk about an invasion of privacy……
Every year I go to the same clinic for a mammogram. And every year I have to write on the form the date of my last mammogram. I know I might have had one elsewhere since then, but they could list the previous date and ask if I had another one in the interval.
I’ve decided life is too short for this crap. I’ve been with the same healthcare plan for over 35 years, and had the same doctor for 21+. Everything is online so if I’m handed a clipboard with pages of questions I put a line through everything and write SEE FILE in the margin and leave the remainder blank. If I have a new issue I will mention it at the end. I’ve never had anyone refuse to accept it.
My friends in Denmark just swipe their ID card (with a magnetic stripe like a credit card), and that’s it! Even for a referral to a specialist, ascanning service, or a hospital.
The one exception is if they’re actually entering surgery, when they are required to sign their name to a simple up-to-date REstatement of what they are and aren’t allergic to.
I hate when they ask for previous address. I’ve lived in the same house for 65 years. I don’t even remember my previous address because I was just a toddler. I know the street name, but not the house number because my parents were living in a cold water flat on top of the garage of an apartment building. There was a housing crunch back then.
