I actually had that problem over 50 years ago in the Navy on board a large ship.

My shop was out of 1250 forms.

    See: http://navyadvancement.tpub.com/12024/css/Single-Line-Item-Consumption-Requisition-Document-Manual-Navsup-Form-1250-1-129.htm

And I needed a 1250 to requisition a box of them.

I finally got a storekeeper to give me ONE out of a box in his storeroom as long as I promised to be back ASAP with it properly filled out and signed to pick up that box.

    Which I did.

Reminds me of Henry and Radar on M*A*S*H

“What am I signing, Radar?”“These are the forms to get the forms that enable us to order more forms, Sir.”

An old Navy trick (which some paople missed): When you get a box of req. forms, fill one out for more forms and place it near the bottom of the box.

When I was an engineer at Boeing we had to fill out a “one-page test request” whenever we wanted to do wind tunnel aircraft tests. Sure enough, eventually I found a continuation page.

I had a supply clerk U. S. Navy) who, for short while, tried to require such forms for pens and pencils.

Forms, forms and more forms. I once worked for a large company that absolutely loved forms. We even had to fill out a form at the end of the week that stated we filled out all of the forms for the week. Thank goodness for middle management. I have to go take some aspirin now.

I needed to renew a permit to posses specimens for our museum collection. Government wouldn’t sent the permit until it had been paid for; university wouldn’t pay until the permit had been received…
