The usual way. You drill a small hole, then expand it with a keyhole saw until there is room for larger saw.

Picture the artist trying to position all of the elements of the cartoon. It worked out best for the artist to have the carpenter behind the wood which made the saw teeth face the carpenter, so he had to have the saw start in the middle of the wood even if normally one would start sawing on the edge. Having sawed wood and drawn comics, I have an eye for this kind of detail.
