Perhaps it’s the safety equipment itself leading to the reckless behavior that causes the injuries. If you weren’t wearing a helmet, would you still lead a tackle with your head?

Rugby and Football are different by nature. American Football has head to head tacking. Also, there is a bit of truth, I think, to the idea that the protective equipment encourages more dangerous behavior.

No, they don’t want to end up paralyzed due to getting hit on the head or from other head/neck injuries.
This is why, in US Football, that a legal tackle occurs from the chest downwards. If said tackler comes anywhere near the head, it’s a penalty. (Refs watch for that.)

All because it has happened in the past. It’s the same reason why NASCAR drivers are basically strapped into the seat of their car, and why the outer body of said car comes away so easily – to protect the human inside.
