I don’t see any mental health issues here, merely a lack of good parenting to teach proper behavior and eating habits. Unless perhaps the rhyme is meant to remind us of the existence of other cultures around the world which do not use knife and fork in the European style… but still eat Christmas pie.

Like many nursery rhymes, it seems random until you think to look up the history — and find it was made for adults.
